Health care promotion campaign

A shared website of just two pages, two e-mail forms and some downloadable forms. This website is part of a health awareness promotion campaign for companies to reduce illness rates and associated costs (with minimal 15%!). The website is used by the commercial promotion team as a quick information reference for customers.
